The CH01 form can only be used to register changes to a director's name or address.
There is an Activity Workflow that is set up in GEMS which causes an Activity record CH01 form to be automatically created once a transaction happens on the following Screens:
Person Basics
Privileged Information
Name History
Address
Service Address
Once the transaction has been carried out in GEMS, open the Activity from the Notifications area or your Compliance Tracker.
Click Generate.
The CH01 Filing Module will launch in a pop up, click Validate to check your data is correct and if the "Status" is "Valid" then click Submit to file your CH01 Form.
