CH01 Form

The CH01 form can only be used to register changes to a director's name or address.

There is an activity work flow that is set up in GEMs which if active then the CH01 form is automatically created once a transaction happens in the following transaction screens:

  • Person Basics

  • Privileged Information

  • Name History

  • Address

  • Service Address

Once the transaction has been carried out in GEMs, a Compliance Tracker- New Activities pop up will appear. Click Generate.

The CH01 Filing Module will launch in a pop up, click Validate to check your data is correct and if the "Status" is "Valid" then click Submit to file your CH01 Form.
